Mechanism of action
- Aromatase is the enzyme that converts androgens (such as testosterone and androstenedione) into estrogens (estradiol and estrone).
- Exemestane acts as a false substrate: it is recognized by aromatase and converted to an intermediate that covalently binds to the enzyme, leading to its permanent inactivation.
- This results in a profound reduction in circulating estrogen levels. In medical settings, it can lower plasma estrogen by up to 85% or more in postmenopausal women.
- It does not directly interact with the androgen receptor and has no anabolic or androgenic effects. However, because it blocks estrogen synthesis, it indirectly influences the hormonal balance, often leading to an increase in endogenous testosterone levels in men (as is true for all AIs).
Medical uses
In legitimate medicine, exemestane is approved for:
- Adjuvant treatment of early-stage breast cancer in postmenopausal women with estrogen-receptor-positive tumors.
- Treatment of advanced breast cancer in postmenopausal women whose disease has progressed despite antiestrogen therapy (e.g., tamoxifen).
It is not used in men, premenopausal women, or for any androgen-related indication. It is also not approved for fat loss or aesthetic purposes.
Use in bodybuilding and athletics
- Aromex is rarely used for legitimate medical reasons by athletes; instead, it is popular among individuals using anabolic steroids that aromatize (convert to estrogen), such as testosterone, methandienone (Dianabol), or nandrolone.
- The purpose is to prevent or reverse estrogenic side effects, including:
- Gynecomastia (male breast tissue growth)
- Water retention and bloating
- High blood pressure due to fluid retention
- Fat accumulation in estrogen-sensitive areas
- Typical dosage in non-medical use is 12.5–25 mg per day (or every other day), taken orally. This is lower than the medical dose for breast cancer (25 mg daily), but still effective.
- Because exemestane is a steroidal suicidal inhibitor, it can exert prolonged effects even after stopping; blood estrogen levels may take several days to recover.
Side effects and risks
- Estrogen depletion: In men, a dramatic reduction in estradiol can be detrimental. Estrogen is required for bone density, cognitive function, lipid metabolism, and libido. Chronic use without cause can cause:
- Joint pain and bone density loss
- Lethargy, depression, and mood disturbances
- Lowered HDL cholesterol and worsened lipid profile
- Erectile dysfunction and reduced libido (although some users report the opposite initially)
- Adrenal suppression: Exemestane can reduce cortisol synthesis in rare cases, leading to weakness, low blood sugar, and hypotension.
- Hepatotoxicity: While exemestane is not a 17α-methylated steroid, it is processed by the liver. At high doses, transient liver enzyme elevations have been reported, though it is generally considered less liver-toxic than oral AAS.
- Osteoporosis: Long-term estrogen suppression accelerates bone resorption, increasing fracture risk.
Legal and regulatory status
- Exemestane is a prescription drug in most countries (e.g., United States, UK, EU, Canada, Australia). It is not a controlled substance (not scheduled as an anabolic steroid), because it has no direct anabolic or androgenic effect.
- However, possessing, importing, or distributing Aromex without a prescription is illegal in many jurisdictions.
- In the United States, exemestane is FDA-approved and available by prescription. Aromex specifically is a foreign brand; importing it for personal use is a violation of FDA regulations.
- In sport, aromatase inhibitors are not currently prohibited by WADA, but they may be scrutinized if used to manipulate testosterone-to-epitestosterone (T/E) ratios or to mask steroid use. They are banned in some sports organizations but are not on the standard WADA Prohibited List.
Important caveats
- Aromex is a drug with real pharmacologic action. It should not be taken casually to “lower estrogen” without a known estrogen-related pathology.
- In bodybuilding contexts, improper dosing can cause serious long-term health issues (bone demineralization, joint pain, depression).
- Black-market Aromex tablets may be counterfeit, misdosed, or contain other drugs. No quality control exists.
- It is not a “post-cycle therapy” (PCT) drug in the traditional sense—it does not restore natural testosterone production. It is commonly used during a cycle, whereas nolvadex/clomiphene are used for PCT.
