Alphabol is a commercial brand name for methandienone (also spelled methandrostenolone), a synthetic anabolic-androgenic steroid (AAS) developed in the 1950s. It is most commonly known by the original trade name Dianabol (Ciba). Alphabol is typically manufactured by pharmaceutical companies in countries where the drug is still produced for grey-market or controlled distribution, most notably by Alpha Pharma (India). It is an oral steroid, not an injectable.

Chemical identity and structure

• Structural notes: Methandienone is a derivative of testosterone with two key modifications:

Mechanism of action

Methandienone acts as an androgen receptor (AR) agonist. It binds directly to the androgen receptor and modulates gene transcription, leading to:

• Increased protein synthesis and nitrogen retention in muscle tissue.
• Enhanced glycogenolysis and improved muscle glycogen storage.
• Increased appetite and improved overall a positive nitrogen balance.
• Some degree of aromatization: unlike 1-testosterone or trenbolone, methandienone does convert to estrogen via the aromatase enzyme (primarily to 17α-methylestradiol). This gives it both estrogenic and androgenic effects.

Its anabolic-to-androgenic ratio is roughly 100:40 (compared to testosterone at 100:100), meaning it is moderately more anabolic than androgenic, but the distinction is not clean in practice.

Clinical / historical context

• Methandienone was developed by Ciba Laboratories in the late 1950s and introduced as Dianabol in 1960. It was originally used to treat conditions such as osteoporosis, burn victims, and certain catabolic states.
• It was widely used by athletes and bodybuilders in the 1960s–1980s and became the archetypal “bodybuilding steroid.”
• Over time, its legitimate medical use dwindled due to the availability of safer alternatives and recognition of its liver toxicity, lipid effects, and suppression of natural testosterone. Today, methandienone is not approved for human use in most developed countries (including the US, UK, and most of the EU). It is still produced commercially in some countries (e.g., India, parts of Eastern Europe, and Latin America) for limited prescription use, but a substantial amount of what is sold in the black market comes from underground labs.

Alphabol specifically

Alphabol, as a branded product, is most commonly associated with Alpha Pharma. The typical formulation is 10 mg tablets of methandienone, in blister packs. It is presented as a pharmaceutical-grade oral anabolic steroid. The brand itself is not a distinct chemical—it is simply a marketing name for methandienone, and the effects/side effects are identical to those of any other methandienone product (e.g., Dianabol, Danabol, Metandienone).

Effects (pharmacological)

• Anabolic effects: Promotes significant gains in muscle mass and strength over short cycles (4–8 weeks), largely via increased protein synthesis and water/glycogen retention. Users often report rapid weight gain (5–10 kg) in the first few weeks, much of which is intracellular water.
• Androgenic effects: Can cause acne, oily skin, accelerated hair loss in genetically predisposed individuals, and increased libido.
• Estrogenic effects: Due to aromatization, it can cause water retention, high blood pressure, and gynecomastia (male breast tissue growth) unless controlled with aromatase inhibitors or SERMs.

Side effects and risks

• Hepatotoxicity: The 17α-methyl group makes methandienone liver-toxic. Elevated liver enzymes (ALT, AST) are common; prolonged use can lead to cholestatic jaundice, hepatic lesions, and in rare cases, liver tumors.
• Cardiovascular: Negative effects on the lipid profile—reduces HDL (“good” cholesterol), increases LDL and blood pressure, and increases platelet aggregation. This raises long-term risk of heart attack, stroke, and cardiac hypertrophy.
• Endocrine: Suppresses natural testosterone production (hypogonadotropic hypogonadism), often resulting in testicular atrophy. Post-cycle therapy (PCT) with SERMs (e.g., clomiphene, tamoxifen) is typically needed to restore androgen production.
• Water retention: Estrogen-mediated fluid retention can elevate blood pressure and cause a “puffy” appearance.
• Gynecomastia: Directly related to aromatization; can require surgical removal if left untreated.
